Simple Steps to Completing the Cremation/Burial Documents.
Page 1 & 2- Vital Statistics, this information is used to complete the personal portion of the death certificate. As you complete these pages, it transfers needed information to the following pages.
Page 3- Disclosure of Preneed Funeral Agreement, when printed, sign were indicated. We will complete the form when our files are searched, if we do have a preneed on file, we will provide you with copies of all paperwork.
Page 4- Release for Transfer, when printed, sign where indicated. NOTE: If the decedent is at the Santa Clara County Coroners Office, you must print out and complete the "Coroners Release" by selecting the choice on the left.
Page 5- Authorization for Disposition with or without Embalming, left click on DO or DO NOT request embalming. We do not embalm for cremation. Fill in the date, city and state below the Sign Here line. When printed, sign where indicated.
Page 6- Declaration for Disposition of Cremated Remains, fill in line 4 "Following manner... If at residence, include name and address, or cemetery name and address, or location of scattering. When printed, sign where indicated.
Page 7- 8- Authority to Cremate, If the urn is to remain at Alta Mesa, initial #2, otherwise initial #3 for Byrgan to continue with your instructions. Leave Fees area blank as we pay the charges for you, Page 2 of Authorization- legal next of kin to sign and print full name.
Page 9- This page is for you information only and is part of the form of page 6.
